Maui County had four clusters of COVID-19 cases under investigation as of Thursday, including none in the travel sector for the first time in weeks.

There were two clusters totaling 27 cases in the construction and industrial sector, as well as one cluster of 14 cases in educational settings and one cluster of four cases tied to restaurants, according to the state Department of Health’s weekly cluster report.

Clusters among restaurant workers have been identified as restaurants open up and serve more local customers and tourists, the report said. Servers interact with the public and kitchen staff and often work in hot, crowded spaces with poor ventilation.

In May, the department investigated a cluster of six cases associated with a restaurant on Molokai, the report said. Four of 35 employees tested positive for the virus while another employee developed symptoms consistent with COVID-19 but was not tested, for an attack rate of 14 percent. Attack rates refer to the proportion of people who get sick after an exposure, according to the World Health Organization.

A close contact of an employee also developed symptoms consistent with COVID-19 and tested positive, bringing the total cluster to six epidemiologically linked cases, the report said. None of them had been vaccinated for the virus, and none were hospitalized.

“Although the restaurant required employees to wear masks in all areas, mask compliance was low among kitchen workers due to the heat,” the report said. “Employees also carpooled to work; carpooling has been associated with an increased risk of transmission across workplace settings. Finally, employees had low vaccination coverage although COVID-19 vaccine has been widely available on Molokai.”

The report said that five employees, or 14 percent, had been vaccinated, and that none of the vaccinated employees tested positive or developed symptoms.

DOH was also investigating five clusters in Honolulu County, including two clusters of 44 cases connected to social gatherings, one cluster of 35 cases linked to a correctional facility and two uncategorized clusters of 44 cases.

Hawaii County had one cluster of 89 cases at a correctional facility, while Kauai County had none.
